Complete this form for a chance to win! Each month we start fresh on the 1st, so set yourself a reminder to reenter every month. We'll randomly select a winner for each ASCEND location on the 1st of each month.
Is There Any Fine Print?

Only one submission per person per month is permitted, but that doesn't stop your other coworkers from also getting involved!
Event must be scheduled during the month you win. We'll promptly reach out to you to schedule a day and time!
Rental shoes and chalk are included, but not harnesses. Extra coworkers may attend your event, but at standard admission rates. Depending on how many people in total, discount at restaurant partner may vary.
